We're seeking an LVN to join our healthcare team. The perfect candidate will possess expertise in home health, wound care, geriatrics, and outpatient services. Responsibilities include coordinating patient care plans, scheduling appointments, and offering support to patients and families. This role calls for a compassionate individual with stellar communication skills and the capacity to engage effectively with diverse patient populations.

Responsibilities:

  • Coordinate patient care with other healthcare professionals.
  • Schedule and coordinate home health visits, wound care, and other services for patients.
  • Provide patient education on home health care, wound care, geriatrics, and outpatient services.
  • Assess patient needs and provide appropriate referrals to other healthcare providers.
  • Maintain accurate records of patient visits and treatments.

Qualifications:

  • Valid Licensed Vocational Nurse (LVN) license in the state of CA.
  • Minimum of 1 year of nursing experience, preferably in home health or a related field.
  • Strong clinical skills and knowledge of various medical procedures.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and manage time effectively.
  • Compassionate and patient-centered approach to care.

Job Types: Part-time, Per diem

Pay: $40.00 - $55.00 per hour

Expected hours: 5 – 20 per week
